The health-care reform law, aka Obamacare, is poised to enter a crucial 12 months that includes setting up state exchanges, levying taxes on the wealthy, and preparing for the individual mandate.

Obamacare lives. That may be one of the most sweeping consequences of the 2012 presidential election. By winning a second term, President Obama has essentially ended Republican hopes of rolling back his signature Affordable Care Act before it takes full effect. Even adamant opponents of the health-care reform law in the GOP-controlled House recognize that it's pointless to continue pushing repeal legislation, at least for now.

"Obamacare is the law of the land," said House Speaker John Boehner (R) of Ohio on Nov. 8.

With its immediate future secure, the health-care law now enters a crucial 12 months. Federal and state officials must race to prepare for Jan. 1, 2014, which is opening day for some of the Affordable Care Act's most far-reaching provisions. As Health and Human Services Secretary Kathleen Sebelius noted in congressional testimony this past spring, "2013 will be a critical year for building the infrastructure and business operations" needed to expand health insurance coverage to millions of low-income Americans.

It's possible that some aspects of Obamacare (as many people, including the president, now call it) could be scaled back in budget negotiations intended to avert the "fiscal cliff" crisis. But here's a primer on top upcoming milestones as the law currently stands.

State insurance stores. By 2014, each state (and the District of Columbia) is supposed to have a health insurance exchange up and running. Basically, these will be health insurance stores ? markets intended to provide a more organized and competitive way for people to buy a product that's often complicated and confusing.

Those eligible to shop at the exchanges will include people whose employers don't offer affordable health coverage, and others who need to buy insurance on their own for some reason (such as, they're self-employed, or they're retired and not yet eligible for Medicare).

States are supposed to tell Washington by Dec. 14 whether they'll run their own exchanges, partner with the federal government, or not get involved, in which case Uncle Sam will set up and run insurance stores for them.

Whoever controls them, there's a lot of work to get ready for October 2013. That's when exchanges are supposed to open their (presumably online) doors and allow potential customers to check out their products and begin the selection process. Between now and then, exchange officials will have to sort and certify offerings from insurance providers, while building the information systems and electronic customer interfaces they'll need to conduct business.

Adapting fish defenses to block human infections: Antimicrobial peptide of fish gills inspire clean surfaces

ScienceDaily (Nov. 29, 2012) ? An undergraduate research team analyzes peptides from fish gills to engineer antimicrobial surfaces for food preparation and medical devices.

Living in an environment teaming with bacteria and fungi, fish have evolved powerful defenses against waterborne pathogens, including antimicrobial peptides located in their gills. Undergraduate researchers at Worcester Polytechnic Institute (WPI) are studying the biology and the mechanics of one of those peptides with the hope they can use that knowledge to create engineered surfaces that kill bacteria responsible for foodborne illnesses and hospital-acquired infections.

The research team, led by Terri Camesano, professor of chemical engineering, reports its latest findings in the paper "Creating Antibacterial Surfaces with the Peptide Chrysophsin-1," published online in October by the journal ACS Applied Materials & Interfaces.

"Fish have a wonderful solution for blocking bacterial and fungal infections," Camesano said. "In this study, we are working to better understand the biochemical mechanics of that process."

As fish filter water through their gills to extract oxygen, antimicrobial peptides (AMPs), including Chrysosphin-1, trap and kill pathogens before they can invade the fish's bloodstream. Scientists in many laboratories around the world are actively exploring the potential use of these molecules to prevent human infections. In the current study, the WPI team attached AMPs to silicon and gold surfaces using two different approaches and measured the bound peptides' ability to kill the bacterial pathogen E. coli.

In the first method, the AMPs were absorbed directly onto gold and silicon crystals, forming a single layer of molecules with the AMPs lying flat on the surface. In the second method, the tips of the AMPs were attached to the surfaces with a glue-like substance so that the peptides rose vertically, like blades of grass extending up from the ground. Surfaces with both AMP configurations were cultured with E. coli cells. The results showed that when the AMPs were lying flat they killed 34 percent of the bacteria in the culture, but when they were standing up vertically they killed 82 percent.

"The hypothesis is that when peptides are attached vertically to the surfaces, they are better able to move and bend so they take on a shape that is more effective in binding to and disrupting the E.coli cells," Camesano said.

In addition to gathering data about the antibacterial efficacy of the attached AMPs, the WPI research team developed a technique for monitoring, in real time, the attachment of AMPs to surfaces. Using quartz crystal microbalance with dissipation monitoring (QCM-D), the team measured the quantity of AMPs that successfully attached to the surfaces in the horizontal and vertical orientations and the density of the AMP layers, along with other properties.

"This was a powerful process, to be able to essentially watch the binding process as it happened," Camesano said. "It is a technique that we will continue to apply in further studies."

Camesano said gold and silicon surfaces were selected for the current study because their chemical properties are well-suited for AMP binding. In ongoing work, Camesano's laboratory will continue to characterize the mechanics of AMP binding for optimal antimicrobial activity and test other materials, including titanium, stainless steel, and plastics, that would have greater utility in food preparation and healthcare.

"What is also notable about this study is that it is the work of undergraduates," Camesano said. "They've done excellent work here that will inform future graduate studies in our lab."

The WPI undergraduates who co-authored the AMP paper are Ivan Ivanov '12 (chemical engineering), Alec Morrison '12 (biochemistry), and Jesse Cobb '12 (chemical engineering). Co-author Catherine Fahey, a student at George Washington University, worked on the study during at a summer project at WPI. Their work was sponsored in large part by a grant from the National Science Foundation's Research Experiences for Undergraduates (REU) program.

There's no details on hardware pricing just yet, but the online component is free for the first year (JPY 420 / about $5 per month there after). Money no object for your mutt? Sadly only available in Japan for the time being, so you'll need to include a return ticket in that splurge too.

Foreclosure activity remains strong in, around Tri-Cities

The number of local homes entering foreclosure appeared to have ebbed slightly in October, even as foreclosure activity appeared to remain at high levels not seen since 2010.

According to data supplied by foreclosure tracking company RealtyTrac, home foreclosure activity in and around the Tri-Cities remained high in October.

RealtyTrac?s data indicated that 203 homes were moving through the foreclosure process last month within a nine-ZIP code region that includes the Tri-Cities, South Elgin, Elburn, North Aurora, Sugar Grove, Maple Park and much of Campton Hills.

Compared to October 2011, when 170 homes were under foreclosure in that region, foreclosure activity in the region increased 19.4 percent, RealtyTrac?s data said.

Within the four ZIP codes that cover St. Charles, Geneva, Batavia and portions of Campton Hills and surrounding unincorporated areas, foreclosures increased more rapidly. In those four ZIP codes, the number of home foreclosures climbed 40 percent from 77 homes in October 2011 to 108 last month, RealtyTrac said.

The local foreclosure spike was the highest activity seen in October since 2010.

At that time, the Tri-Cities and Campton Hills logged 114 foreclosures, RealtyTrac said. And the nine-ZIP code region suffered 249 home foreclosures. The spike in October continued a trend of escalating foreclosure activity dating back to earlier this year.

However, the rate at which foreclosure activity is increasing appears to have ebbed slightly in October, compared to the previous three months.

In the third quarter of 2012, which includes the months of July, August and September, foreclosure activity in the Kane County nine-ZIP code region surged by 46.6 percent, versus the third quarter of 2011.

In those three months in 2011, the region suffered 470 foreclosures. In that same period this year, the region logged 689 foreclosures, RealtyTrac said.

The rate of increase in the third quarter was slightly less in the Tri-Cities, as foreclosures jumped 34.5 percent, from 246 in 2011 to 331 in 2012. But all the third quarter numbers were higher than even the third quarter of 2010, when the nine-ZIP code region suffered 629 foreclosures, and the Tri-Cities accounted for 317 of those.

While the number of foreclosures has increased, distressed properties continue to find buyers.

Even if its design is a bit... bland.

Bon Jovi, Grohl added to Sandy benefit concert

NEW YORK (AP) ? Jon Bon Jovi, Dave Grohl and Eddie Vedder are adding to the star power at next month's Superstorm Sandy benefit concert at Madison Square Garden.

A news release says the three rock stars will join Bruce Springsteen & The E Street Band, Paul McCartney, Kanye West, Roger Waters, Alicia Keys, The Who, Billy Joel and others on Dec. 12 to raise money for the Robin Hood Relief Fund to benefit devastated areas of New York, New Jersey and Connecticut. Those areas were especially hard hit by Sandy earlier this month, killing dozens and causing billions of dollars in damage.

Tickets for the concert, dubbed "12-12-12," go on sale Dec. 3 at 12 p.m. EST.

(AP) ? Iran lashed out Friday at Turkey for requesting NATO to supply it with Patriot surface-to-air missiles to deploy along the border with Syria, denouncing the step by Ankara as counterproductive.

Iranian parliamentary speaker Ali Larijani made the remarks after a visit to Damascus, a show of support by Tehran to its increasingly diplomatically isolated ally.

"The internal crisis in Syria cannot be solved through the deployment of such weapons," Larijani, who is close to the Islamic Republic's supreme leader, said at a news conference in Beirut where he went after leaving Syria.

Turkey's request earlier this week follows several incidents in which violence has spilled across the border from the civil war in Syria, frequently mortar rounds falling a short distance inside. Patriots would be useful in intercepting ballistic missiles ? a much more serious but still hypothetical threat.

NATO said Wednesday it will consider the request "without delay."

The Syrian Foreign Ministry also criticized the Turkish move, calling it "a new provocative step."

Larijani, who met Syrian President Bashar Assad in Damascus earlier Friday, said Iran was seeking a peaceful solution to the war in Syria.

"The difference between us and the others when it comes to Syria is that the others want to impose democracy through weapons," he said. "Iran cannot accept or support such a way."

"I don't think democracy can be achieved through rocket propelled grenades," he added.

Iran is Assad's strongest ally in the region, and anti-government activists accuse Tehran of sending both weapons and fighters to Syria.

Syria's conflict erupted in March 2011 with an uprising against Assad's regime, inspired by other Arab Spring revolts, but quickly morphed into a civil war that has since killed more than 40,000 people, according to activists.

In violence around Syria Friday, Islamic extremists, including members of the al-Qaida-inspired Jabhat al-Nusra group, battled with pro-government Kurdish gunmen in the northern town of Ras al-Ayan near the border with Turkey, activists said.

The Islamic militants entered the town earlier this month and have since clashed almost daily with the Kurdish gunmen. Both factions add to the complexity of Syria's conflict.

When government forces withdrew from Kurdish areas in northeastern Syria in July, they were quickly replaced by Kurdish fighters from the Kurdish Democratic Union Party, or PYD, who would then battle rebels when they pushed their way into predominantly Kurdish areas. The Kurdish group is affiliated with the PKK, rebels fighting for autonomy in the Kurdish-dominated southeast region of Turkey.

The Islamic militants, who are fighting on the side of the rebels, have played a bigger role in the Syrian conflict in recent months and many openly say they want to set up an Islamic state. The opposition is split, with some groups strongly opposed to the influence of extremists.

In Damascus, regime forces shelled the neighborhoods of Tadamon and Hajar Aswad, where rebels and government troops have clashed for weeks, and also raided the central Damascus neighborhood of Bab Sreijeh, arresting several people there, said the Observatory.

In the nearby Palestinian refugee camp of Yarmouk, activists said a bomb blast killed four people and seriously wounded a member of a faction that has backed Assad in the country's bitter civil war.

The explosion late Thursday was a bomb placed under the car of a member of the Popular Front for the Liberation of Palestine-General Command, the Palestinian group said. It blamed the rebel Free Syrian Army for the attack.

Yarmouk has been pulled into Syria's fighting before, most recently earlier this month when clashes in and around the camp killed and wounded dozens.

When the unrest began, the country's half million Palestinians struggled to stay on the sidelines. But in recent months, many have started supporting the uprising although they insisted the opposition to the regime should be peaceful. Others, like the PFLP-GC, stood by Assad. Earlier this month, the PFLP-GC clashed with anti-government Palestinian gunmen in Yarmouk.

Before meeting with Assad, Larijani met with leaders of the PFLP-GC and other pro-Assad Palestinian groups and discussed the latest Israeli-Hamas spasm of violence in Gaza that ended with an Egyptian-brokered truce on Wednesday. He pledged more Iranian assistance to Palestinians there, said Khaled Abdul-Majid, a Damascus-based Palestinian official who attended the meeting.

Speaking in Lebanon a few hours later, Larijani congratulated the Palestinian people for their "victory" in Gaza which he described as a "tsunami" against Israel.

Elsewhere, the Observatory said the body of Syrian novelist Mohammed Rashid Roweily was found late Thursday in the eastern city of Deir el-Zour, nearly two months after he was kidnapped.

State TV said Roweily was "liquidated by terrorists" ? a term the government uses for the rebels. Syrian authorities deny there is a popular uprising in the country and say the rebels are backed by foreign powers that seek to destroy the country.

Ruwiely, 65, was once the representative of Arab Writers' Union in Deir el-Zour and had written several novels. The Observatory said his decomposed body was found along with those of four others, including a retired army officer. All were kidnapped around the same time.

Journalist Bassil Toufic Youssef, who worked for state TV, was shot dead outside his house in the capital on Thursday, the state SANA news agency said.

In other developments, Qatar, which has backed Syria's rebels in the conflict, invited a newly formed opposition coalition to appoint an ambassador to the Gulf state, the Qatari news agency reported.

The broad coalition ? called the National Coalition of the Syrian Revolutionary and Opposition Forces ? was formed Nov. 11 in Qatar under pressure from the United States for a stronger, more united opposition body to serve as a counterweight to the more extremist forces fighting Assad's regime.

The six-national oil-rich Gulf Cooperation Council and the European Union's 27 foreign ministers recognized the Syrian coalition. Last week, France became the first Western nation to officially recognize the council. President Barack Obama said the U.S. isn't ready to recognize the group as a "government in exile" or to arm it.

ScienceDaily (Nov. 23, 2012) ? Using a combination of the new tools of metamaterials and transformation optics, engineers at Penn State University have developed designs for miniaturized optical devices that can be used in chip-based optical integrated circuits, the equivalent of the integrated electronic circuits that make possible computers and cell phones.

Controlling light on a microchip could, in the short term, improve optical communications and allow sensing of any substance that interacts with electromagnetic waves. In the medium term, optical integrated circuits for infrared imaging systems are feasible. Further down the road lies high-speed all-optical computing. The path forward requires some twists on well-known equations, and the construction of structures smaller than the wavelength of light.

Light bends naturally as it crosses from one material to another, a phenomenon called refraction that can be seen in the way a stick seems to bend in water. Illusions, such as mirages in the desert or the shimmer of water on the road ahead on a hot day, are caused by a difference in the refractive index of layers of warmer and cooler air. The new field of transformation optics (TO) uses this light-bending phenomenon in a rigorously mathematical way by applying the 150-year-old Maxwell equations describing the propagation of light onto structures known as metamaterials, artificial constructs with custom-designed refractive indexes. The most famous applications of metamaterials are cloaking devices and perfect lenses, but those are just the tip of the optical iceberg.

In a paper in a new online journal, Light: Science and Applications, published by Nature Publishing Group, Douglas Werner, professor of electrical engineering, and his post-doc Qi Wu and Ph.D. student Jeremiah Turpin present a unified theory for designing practical devices on a single platform using transformation optics. "This field (transformation optics) is in its early stages, so there are many contributions to be made," Werner says. "Our big contribution is in figuring out how to develop TO designs with the simplest material parameters without impacting performance, and linking the devices together to form an on-chip integrated photonic system."

Among their designs are light collimators, which take light from a source and focus it into one or more tightly focused beams, waveguide couplers, which connect different sized waveguides, TO splitters, which divide power from an input waveguide to two or more output waveguides, waveguide crossings, which routes light in compact spaces without loss or crosstalk, and TO benders, which turn the light around corners without loss. These devices are only five to ten microns in size, and therefore many of them could fit on a centimeter-sized chip.

"It has been a joint effort to develop these transformation optics tools and designs," Werner says. "Jeremy wrote the algorithms behind the simulation tools. Qi is developing the designs to be simulated. In order to get the best design for a targeted application, thousands of simulations may have to be performed using powerful optimization techniques developed in our group."

Transformation optics devices that perform diverse, simple functions can be integrated together to build complex photonic systems for optical communications, imaging, computing, and sensing, say the authors. The current, non-TO approach is to design each device using different methods and materials that may not be compatible on a single platform. The Werner group's technique, on the other hand, employs graded index metamaterial structures, such as patterned air holes or rods, on a silicon-on-insulator platform that can be easily integrated into on-chip photonic systems, providing broad bandwidth and low losses.

In order to keep their designs grounded in reality, the group works closely with nanofabricators at Penn State's Nanofabrication Laboratory who are themselves developing new approaches to implement the Werner group's designs. All of the designs described in the paper can be realistically built with current fab processes, says Werner. "It's like a CAD tool," he explains, citing the computer-aided design tools used in manufacturing. "We've developed customized transformation optics simulation and optimization tools for designing optical devices. Beyond that," he continues, "TO is flexible enough that it opens up the possibility of creating all sorts of new devices that don't currently exist."

Novel therapeutic advancement in search for heart muscle progenitor cells: New hope for heart attack patients

ScienceDaily (Nov. 22, 2012) ? Breakthrough in heart research: The research team from Professor Katja Schenke-Layland of the Fraunhofer Institute for Interfacial Engineering and Biotechnology IGB in Stuttgart has discovered cell surface markers that enable the identification and isolation of living functional cardiovascular progenitor cells (CPCs). For the first time, therapeutically relevant CPCs can be derived from induced-pluripotent stem cells (iPS) cells. CPCs, which are typically only found in fetal development, can become all of the different cell types of the heart and can integrate into heart muscle tissue after injection.

An estimated 17 million people die from cardiovascular disease each year. Although mortality rates are declining, heart attacks are still among the most frequent causes of death in the developed world. Often, the cause of a heart attack is the closure of a coronary artery that supplies blood to the heart, which kills heart muscle cells. Cardiomyocytes, which are the heart muscle cells responsible for the contraction of the heart, are not able to regenerate after a heart attack. The massive loss of cells and tissue, and the highly restricted regeneration capacity of the adult heart, lead to an impaired blood supply throughout the body that drastically affects a patient's quality of life. To restore the heart's function after a major heart attack, clinicians require functionally mature cardiomyocytes that perform like the native cells in the adult heart to replace the cells that were killed.

The production of such functional cardiomyocytes from well-defined cardiovascular progenitor cells (CPCs) is the focus of the research team led by Prof. Dr. Katja Schenke-Layland from the Fraunhofer Institute for Interfacial Engineering and Biotechnology IGB in Stuttgart and her colleagues, Dr. Ali Nsair of the University of California Los Angeles (UCLA) and Prof. Dr. Robb MacLellan of the University of Washington in Seattle, who have now succeeded in identifying such cells in a mouse model. The work could revolutionize the treatment of heart disease.

Development of heart muscle cells from precursor cells

Myocardial cells -- as well as endothelial cells and smooth muscle cells -- develop from CPCs during the embryonic development of humans and other animals. There has been a significant amount of research effort towards discovering a path for the clinical application of these cells in patients. The reason for the lack of success is that the markers that help to identify CPCs, such as Islet1 or Nkx2.5, are located in the nucleus of the cells. The use of these cell markers modifies the cells rendering them therapeutically unusable, making the identification of safe cell-surface markers essential.

Surface markers identified for cardiovascular progenitor cells

On this task, the research team of Professor Katja Schenke-Layland from the Fraunhofer IGB in Stuttgart, Prof. Dr. Robb MacLellan and Dr. Ali Nsair of the University of California Los Angeles (UCLA), where Schenke-Layland previously worked before returning to Germany to join the Fraunhofer-Gesellschaft's Attract Program, focused their research. With success: They were able to identify two markers, the receptors Flt1 (VEGFR1) and Flt4 (VEGFR3), on the surface of CPCs with which these cells can be clearly identified while fully preserving their biological function. This discovery allows scientists to isolate clinically relevant cardiovascular progenitor cells that can be functionally matured.

In the search for surface markers, the researchers investigated the cardiovascular progenitor cells using microarray gene expression profiling. These studies show exactly which genes are active at a specific point in time. The resulting data from this analysis were compared to the sequencing data from existing databases of already known as cell markers.

From induced-pluripotent stem cells, cardiovascular progenitor cells are developed

Encouraged by the success of being able to identify and isolate living CPCs, the researchers sought out to derive the cells from induced-pluripotent stem (iPS) cells. For this purpose, they used a method for which the Japanese scientist Shinya Yamanaka was recently awarded the 2012 Nobel Prize for Medicine. This work, published just six years ago, demonstrated that only four proteins are responsible for the embryonic state of cells (Takahashi K, Yamanaka S. Cell 2006, 126 (4): 663 -676). He brought those four genes into differentiated -- mature and specialized -- cells, which then returned them back to an embryonic state. From these cells, which he called iPS cells, scientists can develop all cells of the body, such as liver cells, nerve cells or heart muscle cells.

In their study, the researchers used cells from a mouse strain in which the cells are labeled with a visible green fluorescent protein (GFP) that can be identified with a fluorescence microscope. The cells from these mice were then reprogrammed with the same four genes discovered by Yamanka, resulting in iPS cells that could be easily identified.

In a next step, the researchers cultured the GFP-labeled iPS cells in the laboratory under different conditions with cell-influencing solutions such as growth factors. "Using our newly established cell surface markers, we could detect and isolate the Flt1 and Flt4 positive CPCs in culture," says Schenke-Layland. "When we cultured the isolated mouse CPCs then in vitro, they actually developed -- as well as the embryonic stem cell-derived progenitor cells -- into endothelial cells, smooth muscle cells and more interestingly into functional heart muscle cells."

iPS cell-derived CPCs integrate into the living mouse heart

But how do the developed CPCs behave in living organisms? Can these cells really integrate into tissue and regenerate heart muscle? To answer these questions, the scientists injected the GFP-labeled CPCs into the hearts of living mice. After 28 days, the researchers analyzed the hearts and saw that the green fluorescent cells had developed into beating heart muscle cells and had fully integrated into the myocardial tissue of the mouse.

Enormous potential for heart research

Researchers have long tried to stimulate the regeneration of heart muscle cells. For this purpose, they inject stem cells or stem cell-derived cardiomyocytes into the heart. Although the majority of studies found a slight improvement in heart function, in most cases, neither long-term integration nor the differentiation of the cells into heart muscle has been demonstrated.

The result of the group from Schenke-Layland, Nsair and MacLellan provides the first opportunity to generate functioning heart muscle cells, which integrate into the heart muscle. "We are currently focusing on research with human iPS cells. If we can show that cardiovascular progenitor cells can be derived from human iPS cells that have the ability to mature into functional heart muscle, we will have discovered a truly therapeudic solution for heart attack patients," hopes the scientist.

The work of the research group has been funded by the German-American funding from the Federal Ministry of Education and Research (BMBF) and the California Institute for Regenerative Medicine (CIRM), as well as the Fraunhofer-Gesellschaft (Attract Program), the Ministry of Science, Research and the Arts of Baden-W?rttemberg, and the US National Institutes of Health (NIH).
